Purpose: This survey seeks feedback from PHN CEOs on the effectiveness, value, and impact of the PHN Cooperative's MHAOD Coordination Committee (the Committee).
Why your feedback matters: Your feedback will directly inform the Committee’s 2026–27 priorities, ways of working, and how it supports PHNs in national MHAOD policy and reform processes.
Reporting: survey results will be shared and discussed with CEOs at the May PHN Cooperative meeting.

This short companion survey is completed after you have updated your response to the 2025 National Digital Integration Survey. It does not repeat those questions.
Building on the integration details you have just updated, this survey gathers new information the 2025 survey did not cover — partnerships, the regional intake and referral model in which the IAR-DST operates, service directories, and cross-system data flows and interoperability.
Responses will inform national policy, system design and investment considerations by the DHDA System Navigation Section.
Follow-up: the contact details you provide in Section 1 will be used to make any Stage 2 follow-up inquiries directly with your PHN.
