MHAOD Coordination Committee: Quality Improvement

Purpose:  This survey seeks feedback from PHN CEOs on the effectiveness, value, and impact of the PHN Cooperative's MHAOD Coordination Committee (the Committee).  

Why your feedback matters:  Your feedback will directly inform the Committee’s 2026–27 priorities, ways of working, and how it supports PHNs in national MHAOD policy and reform processes. 

Reporting:  survey results will be shared and discussed with CEOs at the May PHN Cooperative meeting. 

Confidentiality:   

  • Responses will be anonymous.   

  • Reporting will be aggregated.  

  • No identifiable information will be shared.
