Events, Engagement and Partnerships Evaluation

This project has been created to house a number of survey templates to be used when running PHN community and stakeholder engagement events/activities (i.e. events and/or activities where registrations are managed by the PHN in a consistent way). This information is important to collect as it is used to inform our performance, board and DoH reporting.

Standard questions are included below for:

  1. General events/activities related to community and stakeholder engagement (i.e. Aged Care Forum, co-design a service model or intervention/product)
  2. Training/Education (i.e. GPLO/PCLO training events)
  3. Collaboration (i.e. partnership/collaboration is fit for purpose and achieving objectives)

Please COPY and DO NOT EDIT the below survey Templates for your activities. If changing mandatory questions please consult with a member of the Communications or KPP team. Please feel free to COPY surveys and add additional questions as required.

To create a survey:

- click + Add a survey below

- click clone a survey

- select 'Event, Evaluation and Partnerships Evaluation' as the project and the template either 1,2 or 3.

When you publish the survey you can include the link on another project page if you already have one set up, please contact a member of the communications or KPP team if you are unsure how to do this.
