Team Care Referrer Survey

The Team Care program aims to improve the quality of life and the ability to live well at home for people with chronic health conditions.Team Care Coordination (TCC) supports access for general practice referrals, whereas Staying Healthy, Staying Home (SHSH) supports referrals from hospital patients. Your feedback is important and will assist us in understanding how we can continue to improve our service.
