Team Care Coordination/ Staying Healthy Staying Home
The objective of the program is to improve the quality of life and the ability to live well at home for people with chronic health conditions.
The program is a free patient centred service for people with complex chronic health conditions that provides:
health and community information/education
comprehensive assessment and coordination of health, community and social support services.
The program is:
delivered by the phone or face to face in home consultations
staffed by Clinical Nurses
managed by Brisbane North PHN
funded by Metro North Hospital and Health Service.
Team Care Coordination supports access for general practice referrals, whereas Staying Healthy, Staying Home supports referrals from hospital patients.
