To ensure consistency and to improve our ability to respond to enquiries, we kindly request that all enquiries are now submitted through the rediCASE Support Request Form below, rather than via phone or email.
Tracking trends and tailoring training
This centralised approach enables us to capture recurring themes and trends in your requests, allowing us to better understand your needs and provide targeted support.
Furthermore, by analysing these trends, we can develop tailored rediCASE education and engagement sessions specifically for your intake and administrative teams. Provide accurate information: Before submitting, double-check that all required fields are filled in. Make sure all details are correct and up to date to help avoid any delays.
Use the rediCASE referral number: For smoother processing of your enquiry, please always include the referral number. If you do not have it, our team will contact you to verify your identity through security questions to ensure the confidentiality of your client's data and information. Call backs can be requested via this form too.
Avoid personal identifiable information: Please do not include your client's full name, date of birth, therapy session invoice, Medicare card number, or other sensitive details. This form cannot be used to process any referrals to MHAOD commissioned providers.
Mark urgent enquiries: If your enquiry is urgent, please note it on the form so we can prioritise it.
Use this form to submit your enquiry, including requests for new user creation, to the MHAOD Team. We will ensure your enquiry is promptly directed to the appropriate team member.
The referral information of your clients and staff is sensitive. Brisbane North PHN is committed to identifying potential risks and implementing measures to minimize the risk of data breaches.
