Definition of Multidisciplinary Team (MDT):
A multidisciplinary team (MDT) in a general practice setting involves healthcare professionals such as GPs, nurses, pharmacists, allied health professionals, and other specialists working together to deliver coordinated care for patients. Each team member contributes their expertise to enhance patient outcomes, reduce hospital admissions, and ensure comprehensive management of chronic conditions.

The purpose of this survey is to gather insights on how practices are utilizing the WIP-PS incentive to support multidisciplinary care models. The Department of Health and Aged Care were unable to inform PHN on current utilization of WIP-PS due to privacy concerns. Your responses are valuable and will help inform future support initiatives for practices. Your participation is voluntary, and all information provided will be kept confidential. The results will only be used for reporting in aggregate. No identifiable personal or practice data will be disclosed. By proceeding with the survey, you consent to participate and agree that your anonymized responses may be used for analysis and program evaluation.
