Multidisciplinary Care Team Consultation

Brisbane North Primary Health Network (PHN) is seeking people with a lived/living experience of severe and/or complex mental health needs or carers or close supporters of someone with severe and/or complex mental health needs to be part of a consultation on the upcoming Brisbane North PHN Multidisciplinary Care Team project.

The piece of engagement is related to the Mental Health Multidisciplinary Services Grant funding from the Commonwealth, and we are scoping and developing a new initiative - embedding Multidisciplinary Care Team (MDCT) practitioners in general practice to improve access, coordination and outcomes for people experiencing severe and/or complex mental health conditions. This could look like having access to clinical supports social workers, mental health nurses, psychologists, occupational therapists at general practice clinics.

We are seeking passionate individuals with lived/living experience of severe and/or complex mental health needs and/or carers of people with severe and/or complex mental health needs to contribute to an exciting project focused on the development of this new service.

There will be a face to face session held on Friday 16 January from 9.30 am - 12.00 pm at the North Lakes Hotel. The will also be a session delivered online on Monday 19 January from 9.30 am - 12.00 pm via Microsoft Teams.
