The following survey is designed to gain feedback from the Mental Health, Alcohol & Other Drugs (MHAOD) services commissioned by Brisbane North PHN (the PHN) in 2021/2022 Financial Year. The aim of this survey is to provide feedback to the MHAOD team at the PHN regarding our performance in supporting you as commissioned service providers to deliver service across the 21/22 year. We will use this feedback to inform our ongoing relationship management and activities and welcome any feedback and suggestions on how we might improve.
The survey takes about 10 minutes to complete. Responses are anonymous, although it is most helpful if you nominate which service stream you are representing. Please consider your relationship with the MHAOD team at the PHN specifically when answering these questions, this may include Program Development Officers, Program Coordinators, Program Support Officers and Managers.
Results will be collated for review by the internal PHN team and used to improve the way we work. We are looking forward to hearing your feedback!
