The Multidisciplinary Team (MDT) Diabetes Pilot Program is a free service for eligible patients designed to improve access to coordinated diabetes management and MDT care for people living with:
- Pre-diabetes
- Type 1 Diabetes
- Type 2 Diabetes
- Gestational Diabetes
Commissioned by Brisbane North PHN and delivered by Healthy Lifestyles Australia (HLA) in Strathpine, the program provides access to evidence-based MDT diabetes care for those who may otherwise experience barriers to accessing allied health services.
Following a successful first year, Brisbane North PHN is expanding the reach of the MDT Diabetes Pilot Program and inviting Expressions of Interest from local general practices with patients who may benefit from allied health-led MDT care.
Practice eligibilityGeneral practices who meet the following criteria are encouraged to submit an EOI:
- Are not currently claiming the Workforce Incentive Payment (WIP) for an allied health discipline that directly supports diabetes management
- Are located within the Pine Rivers region or surrounding suburbs
- Are a solo or small general practice with a maximum of 5 full-time equivalent (FTE) general practitioners (total FTE may include full-time and part-time GPs)
- Have a Practice Manager with the capacity to support patient referrals and share program information with General Practitioners and practice staff.
Successful practices will be required to complete a Memorandum of Understanding (MoU) with Brisbane North PHN. The MoU outlines the practice’s commitment to:
- Identifying and referring eligible patients to the MDT Diabetes Pilot Program
- If claiming the WIP, sharing what allied health discipline your practice is claiming WIP for
- Nominating an alternative contact person should the nominated contact leave or transition to another role
- Participating in regular Practice Health Checks as part of Brisbane North PHN’s commitment to Continuous Quality Improvement (CQI).
- Providing feedback to support ongoing improvement of the program for participating general practices and current and future patients.
