The purpose of the group is to provide a regular forum that enables participants to share information regarding:
changes and reforms in Aged Care service delivery
share information on service delivery models
identify issues that are affecting service delivery in the community
To work together on selected areas as decided by the group toward improved knowledge and service delivery in service delivery.
This hub is an initiative of the Brisbane North PHN and has been designed to help members share ideas, knowledge and information with each other. It also provides links to meeting agendas, minutes and upcoming events for our group.
