  • The purpose of the deathcare knowledge framework is to articulate the knowledge needed to successfully navigate deathcare for adults from diagnosis of a life limiting condition to two years post-death in the Brisbane North and Moreton Bay region. 

    The framework should enable:

    • a common language regardless of one’s role,
    • an understanding of the systems in place,
    • an understanding of options available for deathcare within the region,
    • attainment of knowledge by mapping available education and training options available locally, and
    • identification of current gaps in education and training supporting deathcare.

    The framework is being developed by a working group in through three tiers of consultation and design. 

    The goal of tier 1 (this tier) consultation is to:

    • confirm concepts to be included in the framework for completeness
    • gather feedback on the proposed structure of the framework body.

  • The purpose of this survey is to express interest in a working/steering group in 2025/26. 

    • Healthcare: deathcare gap working group- Group working to improve continuity of care for people within the North Brisbane and Moreton Bay region when transitioning between health and deathcare.
    • Disability and palliative care steering group-To provide guidance on plans to improve access to at home palliative care for people with disability
    • Palliative CATQIP Support- to provide support to candidates of the Palliative CATQIP
    • Collection of stories working group- To collate stories which educate and amplify the importance of what we do, why and how & emphasise the importance of communication and collaboration.  
    • Pharmacist & prescriber education steering group- Guide work to improve prescriber and pharmacist mutual knowledge and understanding around palliative care through joint education.
    • Compassionate communities steering group- Group guiding the project aiming to commence community development activities to form compassionate communities that enable agency and choice for people with life limiting illness and their loved ones, supported by their community.

    Notes on Sub-Groups

    • membership is voluntary and members can withdraw at any time
    • care is taken to minimise burden on members
      1. PHN staff will undertake the majority of ‘leg work’ under direction and guidance of group members
      2. where possible, work is limited to meeting times only with limited preparation required
    • meetings are scheduled at best time to suit members availability and will be via Microsoft Teams to reduce travel burden
      1. in-person meetings may be scheduled if requested by members
    • subgroups will periodically report back to the wider group at regular meetings
    • subgroup member will be credited in any outputs (e.g. reports, resources etc)
    • membership is not limited to BNPCC members. Additional Subject Matter Experts may also be invited to join the group (e.g. consumers and carers, representative bodies).

    Palliative Connect are events hosted by the BNPCC that are open to all people living and/or working in the North Brisbane and Moreton Bay region who would like to engage, learn, and share to improve palliative care. It is an opportunity for the BNPCC to share its knowledge and engage with others outside of the membership on topics of interest. 

    The Palliative Connect in June has an Aged Care Theme. 

    This event will be 2 hours in duration and consist of: 

    • Didactic presentation/s (~40min)
    • Group activity and discussion (~40min) 
    • Unstructured networking (~40min)

    What questions do you have around aged care? 

    Do you have specific questions around the new Support At Home End of Life Pathway? Do you have questions around workforce planning? Do you ever wonder how something worked in aged care when it came to deathcare? 

    Your questions will guide our discussion and guest speakers for the meeting on 26 August 2025.

    Brisbane North PHN invites expressions of interest from BNPCC members to sit on an evaluation panel. 

    The PHN has requested proposals from select consultants in developing a current palliative care needs assessment for the region. Members of the evaluation panel will independently review and assess proposals received against selection criteria. Members then meet as a panel at a mutually agreeable time/location to discuss and make recommendations to the PHN on successful proposal (~60-90min, may be online).
