Population ageing is reshaping health and care systems, requiring a shift from service-based responses to broader approaches that support people to live well as they age. Metro North Health undertook a collaborative visioning process to develop a long-term aspiration for ageing well to 2041. The process brought together stakeholders from health, aged care, community, government and consumer sectors to identify a future state characterised by 5 key drivers including stronger community connections, prevention and health literacy, integrated and navigable systems, care delivered closer to home, and the thoughtful use of technology to support independence and wellbeing.
The resulting vision provides a shared foundation for future planning across sectors and organisations. The next stage of the work will focus on co-designing a roadmap of tangible actions required to translate the vision into practical system change.
