Practice Support Quality Improvement- Practice Survey

Brisbane North Primary Health Network (PHN) is currently reviewing its quality improvement support activities. We are seeking feedback from PenCAT practices to gain a better understanding of your needs around quality improvement (activities to monitor, evaluate or improve the quality of care delivered). Your feedback will assist us to:

·  understand the current state of quality improvement activities in the region and

·  develop appropriate support services for the region in the future.

The survey should take approximately 5 minutes to complete and your participation is voluntary.

Engagement HQ provides public engagement software to assist us in collecting survey data. Data from this survey will be stored using their server in Australia.
