Integrated Model of Dementia Care Program In Brisbane North – Moreton Bay Dementia Directory Survey

Brisbane North Primary Health Network (PHN), in partnership with the Metro North Hospital and Health Service is currently implementing a 4-year Integrated Model of Dementia Care Program aiming to improve the quality of care for people living with dementia and their carers in the Brisbane North region.
