This survey measures the effectiveness of the training in clinical pathway and communication project evaluation. In particular, the survey seeks to measure whether the project has delivered on its intended objective to provide improved education, communication and clinical support to RACF nurses in select clinical pathways. 

The survey should take approximately 10 minutes to complete. If you experience any difficulties in completing the survey, please contact Brisbane North PHN on agedcareprojects@brisbanenorthphn.org.au.
