Brisbane North PHN is inviting expressions of interest from residential aged care in the Brisbane North and Moreton Bay region to participate in a program of support designed to:
- enhance after hours care
- strengthen telehealth capacity within your facility through provision of funding
- improve engagement and collaboration between health professionals working in residential aged care across Brisbane North
- increase access to training, support and resources
The activities delivered as part of this program can assist your facility to meet the Aged Care Quality Standards (ACQS) through enabling timely access to advice and care and playing a role in continuous improvement in quality of care and service delivery.
