RACH Program Evaluation Survey (2025-26)

Brisbane North PHN delivers several programs for Residential Aged Care to improve digital health, after hours care, access to quality primary care for residents. This survey will enable us to understand the progress we are making in these areas, and plan relevant activities to support RACHs in the 2026 – 2027 financial year.

We are not asking you to identify yourself in this survey, but we are asking you to name the RACH you work in. This will enable us to identify any RACHs who have not completed the survey, and match data on questions that you may not know the answer to (others at you RACH may complete the survey and have this information).

It is important to know that individual RACH data is never shared, and our reporting will only include combined data. For example, we may report that XX% of RACHs in the Brisbane North region use telehealth, but we will not name RACHs that do or don’t use telehealth.

This survey should take approximately 5 minutes to complete. Thank you for taking the time to complete this survey.
