ACOP Input Survey

This survey is for any Aged Care Onsite Pharmacists (ACOPs) in the Brisbane North PHN region to inform the PHN about their experiences as an ACOP, and share their suggestions of what the PHN should focus on in the upcoming year to help support the ACOP rollout. It also provides ACOPs with options to access the (free) services offered by the PHN, which have been designed by the community to support the role.
