Pharmacy - After Hours Needs Assessment Survey 2019

Brisbane North PHN is seeking to understand the after hours health care needs of our community and the Pharmaceutical Society has kindly agreed to help us collect the experience of pharmacists. 

Your responses are voluntary and only aggregate results will ever be reported.  This survey should take approximately 2 minutes to complete.

For the purposes of this survey the after-hours period is defined as after 6pm and before 8am weekdays, before 8am and after 1pm Saturdays and all day Sundays and public holidays.
