Congratulations on completing the Palliative CATQIP!
As this is the first time we have run this program, we would love your honest feedback on your experience. Your feedback will be collated and will help assess the program- to understand if the PHN should continue the program in the future and if so, how it can modified to offer the best experience for candidates.
The survey comprises of 7 questions (4 text questions and 3 tick box questions). All feedback is anonymous and all questions are optional. If you have any concerns, please contact Caroline (caroline.irle@brisbanenorthphn.org.au).
During December, all candidates will asynchronously discuss their progress with their change activity. This is the online submission survey. There is a word template version available if preferred.
