Best Practice Models for Disability Palliative Care and Medication Assistance

We wish to invite you to participate in a consultation on palliative and end of life care delivered by disability service providers. The consultation will help develop a suite of support materials for providers operating in the Brisbane North PHN region. 

You are invited to share your thoughts and experience through participating in this survey which involves a series of 14 open text questions.

• Evaluation

Any raw feedback and identifying information shared will only be accessible to the project team. All results will be deidentified. Participation is voluntary, and you may withdraw consent at any time.
