A five year health care plan for older people who live in Brisbane North (2017-22)

Working in partnership, Brisbane North PHN and Metro North Hospital and Health Service (MNHHS) have developed a five-year plan for the health care of older people who live in Brisbane North (the Plan). The Plan describes directions and actions to enhance the health care system to better support the needs of older people who access health services across our region.

The directions and actions listed in the plan are ambitious and will require a commitment from all health care providers across Brisbane North to deliver.

The PHN and MNHHS have developed a joint project implementation plan outlining the activity, timeframes and resources needed to deliver on the Plan.

The project implementation plan has been split into three phases:

Phase One: 2017/18 to 2018/19

The tasks depicted in this phase will be the responsibility of the PHN and MNHHS.

Phase Two: 2019/20 to 2020/21

The tasks depicted in this phase will be the responsibility of the PHN, MNHHS and other key stakeholders, particularly those from the community and primary care sector. Key stakeholders external to the PHN and MNHHS will be encouraged to become signatories to the Plan. In doing this, they will be asked to contribute at least one task to the project implementation plan for which they will be responsible for implementing and reporting on.

Phase Three: 2021/22

A small number of priority tasks will be actioned in this phase of the PIP, these include:

  • care coordination
  • workforce
  • interface between health and ageing
  • clinical care and governance
  • vulnerable groups
  • wellness and reablement

The PHN, MNHHS and signatories to the Plan will work together to fully develop and implement these tasks.
