Enhancing the health care system to better support the needs of older people

Working in partnership, Brisbane North PHN and Metro North Hospital and Health Service have developed a five-year plan for the healthcare of older people who live in Brisbane North. The directions and actions are ambitious and will require a commitment from all healthcare providers across Brisbane North to deliver.

We want to hear about any healthcare work in the Brisbane North region to:

  • provide care to meet the individual needs of older people
  • support older people to stay healthy, well and independent
  • develop a healthcare system that is connected and integrated, delivering care locally wherever possible
  • provide access to high quality care in our hospital services
  • enhance care for older people who are vulnerable
  • advance continuous improvement in older people’s care

  • New Five Year Healthcare Plan For Older People

    To support our region’s growing community of people aged 65 and over, Brisbane North PHN in collaboration with Metro North Hospital and Health Service (MNHHS), has developed a new five year healthcare plan for older people.

    This five year plan will guide the way healthcare services are delivered to this group of people in our community and represents a commitment between the PHN and MNHHS to improve the coordination, integration and provision of care services to support the health and wellbeing of older people to help them stay healthy and at home.

    Inclusiveness is central to this plan, by ensuring that patients and their families, community organisations and other key stakeholders, work collaboratively with healthcare providers to keep our older population engaged and healthy.

    Over the next five years, this plan will guide a coordinated, networked service system to help as many older people as possible to remain in their home environment. It is not the last word, but a document that will allow the vision to expand and grow.
