Be a part of the Dementia Service Navigation Directory for North Brisbane and Moreton Bay.
Brisbane North PHN is currently working to improve dementia service navigation across the North Brisbane and Moreton Bay region, by creating a dementia service navigation online platform and hardcopy directory.
It is being designed collaboratively with people living with dementia, their carers and families and health professionals, to best meet each of their needs.
This online platform and hardcopy directory will enable easy navigation and direct users to access a range of community-based and national-level services, supports and resources. The portal will act as a one-stop-shop and link users to your service or organisational website and include contact information.
To ensure we have an up-to-date list of services being offered by service providers and organisations in the North Brisbane and Moreton Bay region, we would appreciate you completing the below short survey by Friday 18th of June
To be eligible to be listed on the website and in the hardcopy directory, your service needs to:
have an interest and experience in caring for people living with dementia.
If you have multiple locations within your organisation, please submit a separate survey for each location.
There is no cost associated with being listed on the website and in the hardcopy directory.
