What is the Innovative Models of Care Grant Program?

It supports organisations to trial and evaluate innovative

multidisciplinary models of primary care in rural and remote

areas of Australia (Modified Monash (MM) 3-7 locations).

Key outcome is to deliver proven and sustainable

multidisciplinary primary care models that are evaluated and

can be adapted and implemented in a range of rural and

remote contexts, bespoke to a community.
