Ideas for Norfolk Island Health Expo - Brisbane North PHN Table
over 1 year agoSo that we can determine if we have enough to warrant a PHN table at the upcoming Norfolk Island Health Expo (September 10), please post resources and/or programs that people living on Norfolk Island could use to support their health and well-being.
Please share what information we would need to take to promote the resource/program at the Expo.
***Before posting, please ensure people on Norfolk Island would qualify/be able to access resources/programs.

My health for life is a FREE, six-month program where you work with a health coach to achieve your health goals. It's a State government-funded initiative, delivered by an alliance of health organisations, and is designed to help Queenslanders stay well and lessen their risk of developing conditions such as type 2 diabetes, heart disease, stroke, high cholesterol and high blood pressure. My Health 4 life is designed to complement current care provided by health professionals. It's a practical extension of the advice given by health professionals to their patients, allowing participants to better understand their health risks and develop longer term, sustainable health goals. Choosing either small group sessions, one-on-one phone coaching, or culturally tailored programs, you explore topics such as nutrition, physical activity, understanding your personal risk of developing health conditions and how to make healthier choices a part of everyday life. With the support of your coach, you will also develop an action plan. Whether your goal is to get more active, eat better, stress less, or lose weight, the focus is on building a step-by-step plan to help you achieve your goals over time.
