Lived Living Experience Feedback Form

This form was developed by Lived Experience Australia in collaboration with people who have lived and living experience. It sits within the Brisbane North PHN Lived Experience Representative Lifecycle Kit—a practical tool that supports every stage of a Lived Experience Representative’s journey with our PHN.

As outlined in the Brisbane North PHN Engagement and Partnering Framework, engagement is an ongoing process, and it’s important we regularly check in on how we’re doing. The framework reminds us that:

“Effective engagement and partnering is an ongoing process, therefore it is important to continually evaluate the extent to which the PHN is maximising outcomes as a result of engagement and partnering.”

That’s where this feedback form comes in. It offers a structured way to reflect on what’s working well and where we can do better. By reviewing the feedback we receive, we can better understand the quality of the engagement experience and identify what changes or improvements might be needed for next time.

This commitment to listening, learning, and improving together is what helps us build stronger, more meaningful engagement over time.
