Thank you for participating in our Initial Assessment and Referral Workshop 1 and 2 training 9 months ago.
The purpose of this final survey is to evaluate long-term outcomes from the training and use of the IAR-DST.
Your valuable feedback will help us to plan for future training, and support us to conduct meaningful input and advocacy to the Department of Health on use of the IAR in our region.

The purpose of this survey is to evaluate outcomes from the training and use of the IAR-DST.
The objectives of the training were to:
- Identify principles underpinning the national approach to stepped care
- Implement the IAR Guidance in practice settings, using the domains and the Decision Support Tool to generate a recommended level of care
- Describe the levels of care and determine regional services matched against the levels of care
- Identify the principles related to clinical decision making and consumer choice, and practice following these principles using supported decision-making strategies

- Provide an orientation to the domains, levels of care, and Decision Support Tool.
- Practice application of the IAR in referral, assessment, and intake settings (practical activity using vignette)
