If you are a resident of the North Brisbane and Moreton Bay region, you are invited to participate in Brisbane North PHN's community survey.
If you are a GP, allied health practitioner, nurse or someone who provides a direct front line health service in the North Brisbane and Moreton Bay region, you are invited to participate in Brisbane North PHN's practitioner survey.
Who is Brisbane North PHN?
We are one of 31 Primary Health Networks (PHN) across Australia. We support clinicians and communities in Brisbane’s northern suburbs, Moreton Bay Regional Council and parts of Somerset Regional Council (map).
Our key objectives are to:
- increase the efficiency and effectiveness of medical services for patients—particularly those at risk of poor health outcomes
- improve coordination of care to ensure patients receive the right care, in the right place, at the right time.
One of the PHN's roles is to understand the healthcare needs of our community. We do this this looking at health service gaps and working with community and health services to develop and deliver plans to address them.

What is a Health Needs Assessment?
A Health Needs Assessment is a way of identifying the health and service needs of our community. It helps to determine people's health priorities, how to fairly spread resources, and plan programs or activities that support people's health needs.
Brisbane North PHN is conducting a Health Needs Assessment in partnership with Metro North Health. Metro North Health is the public hospital and health service for the north side of Brisbane (read more about them here). When this project is finished, the full report will be available on the the Brisbane North PHN website.
Purpose
The purpose of the survey is to engage with people who live in our local community and health practitioners to identify important health and service needs. We want to learn more about the health conditions that impact people who live in North Brisbane and Moreton Bay, and what helps or prevents people from achieving good health and wellbeing.
Responses to the survey will be used to build our understanding of:
- people's health needs living in our community
- people's needs that provide health services in our community
- how to best address the health issues that people face
We do this by reading responses along with data about our local population, the availability and location of services, and how they are used
Your responses are anonymous and will not be shared with third party members. They will be treated confidentially and no information will be sourced back to individuals. Responses are intended to provide a broad picture of residents across the North Brisbane and Moreton Bay region.
