Assessment and Planning Review

The Brisbane North PHN is conducting a quality review of the assessment and planning processes used by healthy@home consortium provider organisations to identify opportunities for improvement that can be shared across the Consortium. Results will be shared with the Management Group at the next meeting. Thank you for participating in this important quality improvement activity.

Are care plans regularly reviewed?

Do you have evidence to demonstrate that consumers are involved in care plan reviews?

If indicated in the assessment process, are consumers referred to other providers of care when indicated?
