Brisbane North PHN & Metro North Health Women, Children and Families Clinical Stream Collaborative provides a forum to improve the continuity and integration of quality patient centred care across primary care, community and hospital for Metro North Women, Children and Families Clinical Services in collaboration with Brisbane North PHN and Children's Health Queensland.
Activities include:
- Quality of referrals
- Integration of care
- Models of care including shared care arrangements
- Policies, guidelines, procedures and practices
- Education and support
- Access to services and information about services
- Research opportunities.
Objectives:
- Focus on the patient journey through a model of integrated care
- Support safe and equitable access to services
- Optimise communication between health providers
- Support the implementation of models of shared care which enhance the patient experience and outcomes
- Support opportunities to contribute to the evidence base through ongoing education and research. The parties will work collaboratively on mutual education activities to improve understanding of each sectors contribution to care
- Highlight and escalate information from members of the group in regard to issues that will affect either party or patient care. This includes changes in referral pathways or clinical practices
- Develop robust clinical pathways between GPs, MNH Women, Children and Families Clinical Services and Children’s Health Queensland
- Align care provided in the community with care provided in hospital whilst strengthening partnerships between Primary Care, Brisbane North PHN and the hospital system
