Patient Access and Experience Survey

Please note, your participation in the survey is voluntary, you may choose to not answer any question. Your responses will remain anonymous. Demographic information is collected in this survey. It is acknowledged that participants may feel their identity could be revealed through collection of this information. However, no part of the analysis will seek to identify participants, and your responses will remain anonymous and confidential at all times. Data collected from responses will be accessed and analysed by Neighbourhood Medical and the Brisbane North PHN. Your responses will be combined with others completing the survey and aggregated data shared with the Department of Health and Aged Care.
