EOI - Ear Wax Removal (Irrigation) Workshop

*Each practice can nominate up to one team member. Due to high demand, we may not be able to offer a place to the nominee. Submitting an EOI does not guarantee a place.

By checking this box, you acknowledge and agree that:

1. This opportunity is limited to a specific number of participants and not all nominated applicants will be able to participate.
2. Only one spot is available for each general practice, and the practice has undertaken reasonable enquiries to ensure that only one expression of interest has been submitted. 
3. Brisbane North PHN will undertake a selection process, and the practice may be asked to provide additional information or participate in a selection discussion.
4. Brisbane North PHN may share the nominated participant’s name and other relevant details with the external training provider for the purpose of managing the registration and following up on participation.
5. The practice and the nominated participant(s) will not share registration links or promotion codes with any other person or organisation.
6. The nominated participant(s) will complete a post-event survey to provide feedback upon request by Brisbane North PHN.
7. The practice acknowledges that there is a deadline by which expressions of interest must be submitted, and late submissions will not be considered.
8. If the nominated participant is selected to participate in this opportunity, the practice will notify Brisbane North PHN within a reasonable time if they are unable to attend or complete the opportunity, so that the place may be reassigned.
9. Nominated participants are expected to share key learnings, resources, and relevant knowledge gained from the training with their wider practice team to support ongoing workforce development.
10. Failure to provide reasonable notice of non-attendance, or otherwise failing to attend, may adversely impact the practice’s eligibility for future training or funding opportunities provided by Brisbane North PHN.
