Has Brisbane North PHN provided support to your practice that has led to a positive outcome?


We invite clinicians and practice staff to share how Brisbane North PHN initiatives, support activities, digital health tools, workforce programs or Continuous Quality Improvement (CQI) initiatives have helped their practice improve proactive patient care, strengthen preventive health activities, identify patients for follow-up or enhance day-to-day operations. Sharing your experience can help other practices understand the practical benefits of working with the PHN to support sustainable, data-informed and high-quality care.

All submitted stories will be entered into an annual draw to win a PHN-supplied team reward.
