Brisbane North PHN is seeking Expressions of Interest (EOI) to participate in a consultation activity focused on understanding current frailty management in primary care and mapping frailty-focused interventions available in our region.
This work will help us identify service gaps, barriers, and opportunities to strengthen preventive and early intervention supports for older people. Our aim is to enable older people to live at home for longer, promote healthy ageing, and support the ongoing management of chronic conditions.
A consultant engaged by the PHN will be undertaking consultations to understand current practices, referral pathways, and opportunities to improve frailty assessment and intervention in our region.
Participants will be remunerated for their time.

This survey is for general practitioners, practice managers and practice nurseswho would like to be involved in consultations with Create Health, who has been engaged by Brisbane North PHN to undertake this work.
Your responses, including your contact details, will be shared with the consultant to enable them to coordinate consultation bookings.
Consultations are expected to be scheduled from 19 January until 20 February 2026.
