Brisbane North PHN is seeking Expressions of Interest (EOI) to participate in a consultation focused on understanding current frailty management in primary care and mapping frailty-focused interventions available in our region.
This work will help us identify service gaps, barriers, and opportunities to strengthen preventive and early intervention supports for older people. Our aim is to enable older people to live at home for longer, promote healthy ageing, and support the ongoing management of chronic conditions.
A consultant engaged by the PHN will be undertaking consultations to understand current practices and referral pathways, and strengthen frailty prevention, assessment and intervention across the region.
Participants will be remunerated for their time.
