Part of the PHN Cooperative’s Consent Toolkit

This Consent Flowchart is part of the Consent Toolkit, developed by the PHN Cooperative to support Primary Health Networks (PHNs) in building consistent, practical guidance on consent requirements. It is designed to help PHN staff confidently assess and meet their legal and ethical obligations when handling personal information.

As outlined on page 5 of the Toolkit, the flowchart guides users through the key steps required to meet consent obligations under two common scenarios:

  • Scenario 1 – Direct Collection of data by a PHN

  • Scenario 2 – Indirect Collection, where data is received from another entity (e.g. general practice or commissioned service provider)

While this flowchart has been reviewed by an external privacy compliance consultant, we acknowledge that some scenarios may still be complex. If you’re unsure about a particular step, please seek advice from your Privacy Officer.

For questions, suggestions, or feedback about this tool, you are encouraged to contact a member of your Consent and Privacy Steering Group (you should have been advised of your local representative when the Toolkit was shared with your PHN).
