Dementia Quality Enhancement in Primary Care Project - Post Evaluation Form with Participating Practices 2019

This survey measures the effectiveness of the Dementia Quality Enhancement in Primary Care Project (DQEP).The survey particularly seeks information to measure whether the project has delivered on its intended outcomes, to improve quality of care for people living with Dementia and their carers in the Brisbane North region. Your opinions will assist us in determining if the initiative is responsive to community needs and should be continued and expanded. The survey should only take approximately 5 minutes to complete.
