This course is specifically designed for clinical staff in primary care, including practice nurses, registered nurses (RNs), enrolled nurses (ENs), and general practitioners (GPs), to enhance their skills in managing chronic disease care.

This course will equip your clinical staff with the knowledge to develop patient care plans for chronic diseases, implement health interventions that identify risk factors, and support behaviour changes that lead to improved health outcomes.

Learning Outcomes:

• Determine chronic disease management needs, identify risk factors, and engage patients effectively.

• Support behaviour change, recognise the need for referrals, and ensure follow-up and review.

• Apply effective care planning techniques to enhance the management process.

Students must complete the Foundations of Chronic Disease online module to enrol in this program.
