Dementia Service Navigation Project

We are helping people with dementia, their family and carers, and health professionals to quickly and easily find the support they need in the Brisbane North and Moreton Bay region.

The Dementia Service Navigation portal is a one-stop-shop to find trusted information and support.

The Dementia Service Navigation portal and hardcopy directory aims to:

  • enable easy service navigation and access to information, resources and education.
  • cover the support needs areas of the dementia journey.
  • build knowledge and skills so people living with dementia can remain in the community as long as possible.
  • build the knowledge of Health Professionals about dementia and how they can best support their patients.
  • In time, expand the dementia website to be inclusive and used as a resource for healthy ageing for all Older People in the Brisbane North region.

It is essential that Dementia Advocates be involved in the process.

This project page is accessible by the working group members, including New Word Order and Dementia Australia.

Inclusion and exclusion criteria

To be listed on the Your Care Navigator website, service providers including community, RACF or respite needs to:

  • Have service provided and primary business located in the Brisbane North and Moreton Bay region*

  • have an interest and experience in caring for people living with dementia.

  • If an RACF or service provider meet and comply with Aged Care Quality Standards and be accredited with the Aged Care Quality and Safety Commission (ACQSC). More information is available on the ACQSC website on how to become an aged care provider. ACQSC is the accreditation body for Commonwealth funded services.

    Your home, your health, your way



    "Your Care Navigator" has been chosen as the favourable naming option for the dementia service navigation portal.


    Consumers and carers have given feedback and supported this name.


    We found from people living with dementia, families and carers, and other colleagues at BNPHN they felt: 

    • The "Your" feels empowering and "makes me feel like I am important".
    • The "Navigator" aligns with the successful peer navigator COTA program, that BNPHN will be helping to expand in our region in the next FY.
    • The website will act to help people 'navigate' the aged care system, find support near them and learn about dementia. 
    • Dementia support and resources as the second line tells user what they will be finding on the website.
    • "Your Home" - people want to receive their care closer to what they call as home and stay in the community as long as possible.
    • "Your Health" - encouraging people to manage their health and wellbeing. 
    • "Your Way" - puts the power back to the consumer, and makes them feel that they have ownership. Each journey with dementia is different and people, families and carers want to access services their way. 
    • The tag line - "Your home, your health, your way" also aligns with the BNPHN care coordination team tagline

    To remind you all, this website long term plan is to be expanded into health ageing and others supports (this is why we could not have the term 'dementia' in the title).
