Your feedback is important in assisting us to improve the board reporting going forward.

These questions relate to the Organisational Performance Report prepared each quarter. This report currently includes:

- a case study & spotlight each quarter,

- traffic light reporting on all activities,

- performance information by PHN priority and 

- updates on corporate services activities. 

This survey should take no more than 10mins to complete.
