Brisbane North PHN invites you to share your experiences of community health care. We have a created a 10-15 minute survey asking you about your health and health care needs. There are a total of 15 questions, with two additional questions for health practitioners and an option to enter a prize draw for a $250 gift card.
Background:
Brisbane North Primary Health Network is undertaking a Health Needs Assessment to inform its regional commissioning cycle in 2024-27. This survey gives voice to community experiences with health, including health conditions and health care and is available to anyone residing in Brisbane and Moreton Bay regions.
The survey consists of three key sections (Health Needs, Service Needs and Demographics). There is an option to provide your preferred email address if you would like to enter into a prize draw and hear about subsequent opportunities to provide feedback. Demographic data is used solely to inform the Health Needs Assessment and is stored in Engagement HQ.
This survey will run until June 2024.
