New advance care planning guidelines support person-centred care

The Statewide Office of Advance Care Planning has released new Queensland Health advance care planning clinical guidelines:

• support facilities, services and clinicians to implement ACP processes and develop local policies and procedures
• raise awareness of the unique Queensland Health systems and structures that support ACP across all inpatient and community health settings.
