Immunisation Recall and Reminder Grant 2025

Brisbane North PHN (the PHN) is pleased to provide a limited number of general practices with a grant to help them improve their systems for sending out recalls and reminders for COVID-19, influenza and pneumonia vaccination, as well as making vaccines more available, with the goal of increasing community immunisation rates.

The objective of this grant initiative is to:

  • support general practice immunisation providers in improving awareness and accessibility for vulnerable patient cohorts of COVID-19 vaccines and boosters, influenza, pneumonia and other immunisations recommended under the National Immunisation Program (NIP)
  • coordinate the allocation of nursing resources to enhance recall and reminder activities that aim to improve immunisation rates within practices, and to educate patients on the significance of vaccinations
  • enhance vaccine uptake to support the reduction of transmission for vaccine-preventable diseasesin the community.

About the grant opportunity:

The PHN Immunisation Recall and Reminder Grant of $2,000.00 (excluding GST) has been allocated for general practice vaccine providers within the Brisbane North PHN region.

The aim of this funding is to:

  • enhance the efficiency of practice time allocation and manage expenses related to the provision of reminders and recalls
  • facilitate targeted and opportunistic educational discussions regarding vaccination with vulnerable populations
  • achieve an increase in immunisation uptake for participating practices.

This initiative is in alignment with the National COVID-19 Vaccine Program (NCVP) and the National Immunisation Program (NIP), with a sole focus on vaccinations for COVID-19, influenza and pneumonia.

Participating general practices will be provided with a complimentary PHN-funded GoShare Influenza SMS campaign valued at $500. This bundle includes up to 1,000 SMS credits per practice. Additionally, GoShare will collaborate closely with each practice over the activity to:

  • identify vulnerable and at-risk patients
  • create SMS lists
  • develop SMS templates
  • deliver bulk personalised SMS messages inviting patients to schedule an appointment with their healthcare provider to discuss their eligibility for the Influenza vaccination.

GoShare will provide training on system usage for practice staff, offer relevant resources, and maintain a helpdesk for support.

Additional Activities:

GoShare is also offering complimentary COVID-19 and pneumonia SMS recall campaigns, which include 500 SMS credits per campaign and are fully sponsored by Pfizer.

Data reporting requirements:

General practices participating in the Immunisation Recall and Reminder Grant 2025 will be required to submit pre- and post-activity statistics from their Clinical Information System (CIS) or the Primary Sense Winter Wellness report to complete the data reporting requirements section in your practice's Continuous Quality Improvement (CQI) Plan as part of the activity.

Application process:

The initial step in the application process for the Immunisation Recall and Reminder Grant 2025 is to review the Immunisation Recall and Reminder Grant 2025 Guide to Payment and Funded Activities document. Once you have reviewed the document and wish to participate, please submit the below Expression of Interest (EOI) form.

Please note that the deadline for submittingthe EOI is Friday 16 May 2025 by 5.00 pm.

Brisbane North PHN will review your EOI and notify you of the outcome. If selected, you will receive an email with your practice onboarding package outlining the next steps.
